I. RMU Enrollment Agreement
I hereby enroll at Rocky Mountain University of Health Professions (hereinafter referred to as RMU) in the degree program indicated above. I understand my enrollment agreement is subject to acceptance by RMU and my graduation date is subject to change depending on my timely completion of all program requirements, as less than full-time enrollment (in programs where applicable), and breaks in continuous enrollment, will impact my anticipated graduation date.
Tuition and Fees: I am responsible for the tuition and fees pertaining to the program’s required course of study during the designated enrollment period. Tuition and fees may be subject to an increase each academic year. The tuition and fee amounts provided are estimates based on the prevailing tuition rate and minimum program credit requirements, both of which may be subject to change. The enrollment deposit paid to confirm enrollment in the intended program is non-refundable.
Change in Name, Address, E-Mail Address, or Phone Number: I am responsible for informing the RMU records office (registrar@rm.edu) of any changes in my name, address, email address, or phone number within thirty (30) days of any such change.
Suspension of Services: I understand and agree that RMU may withhold grade reports, transcripts, diplomas, and other services if I fail to pay tuition and fees or break any of my promises under this agreement.
Refund Policies: The Institutional Refund Policy applies to students who drop all courses in a semester, whether through providing notification to the University or ceasing attendance without notification. Students dropping all courses in a semester are considered withdrawn for refund purposes. If the student participates in Federal Financial Aid, the withdrawal is also subject to the Return of Title IV Funds Policy.
Non-Discrimination Policy Statement: RMU does not discriminate or harass on the basis of race, color, national origin, sex, gender, sexual orientation, gender identity or expression, disability, age, religion, genetic marker, veteran's status, or any other characteristics protected by state, local, or federal law, in our programs and activities. RMU will not retaliate against persons bringing forward allegations of harassment or discrimination.
Renewal: I understand and acknowledge that this agreement is executed at the time of my initial enrollment at RMU and that my enrollment in subsequent semesters shall constitute a renewal of the terms of this Agreement, except for the tuition and fees which may be subject to change.
For Residents of California: “The State of California established the Student Tuition Recovery Fund (STRF) to relieve or mitigate economic loss suffered by a student in an educational program at a qualifying institution, who is or was a California resident while enrolled, or was enrolled in a residency program, if the student enrolled in the institution, prepaid tuition, and suffered an economic loss. Unless relieved of the obligation to do so, you must pay the stateimposed assessment for the STRF, or it must be paid on your behalf, if you are a student in an educational program, who is a California resident, or are enrolled in a residency program, and prepay all or part of your tuition.
You are not eligible for protection from the STRF and you are not required to pay the STRF assessment, if you are not a California resident, or are not enrolled in a residency program.
It is important that you keep copies of your enrollment agreement, financial aid document, receipts, or any other information that documents the amount paid to the school. Questions regarding the STRF may be directed to the Bureau for Private Postsecondary Education, 1747 North Market Blvd., Suite 225, Sacramento, California, 95834, (916) 574-8900 or (888) 370-7589.
To be eligible for STRF, you must be a California resident or are enrolled in a residency program, prepaid tuition, paid or deemed to have paid the STRF assessment, and suffered an economic loss as a result of any of the following:
- The institution, a location of the institution, or an educational program offered by the institution was closed or discontinued, and you did not choose to participate in a teachout plan approved by the Bureau or did not complete a chosen teach-out plan approved by the Bureau.
- You were enrolled at an institution or a location of the institution within the 120 day period before the closure of the institution or location of the institution, or were enrolled in an educational program within the 120 day period before the program was discontinued.
- You were enrolled at an institution or a location of the institution more than 120 days before the closure of the institution or location of the institution, in an educational program offered by the institution as to which the Bureau determined there was a significant decline in the quality or value of the program more than 120 days before closure.
- The institution has been ordered to pay a refund by the Bureau but has failed to do so.
- The institution has failed to pay or reimburse loan proceeds under a federal student loan program as required by law, or has failed to pay or reimburse proceeds received by the institution in excess of tuition and other costs.
- You have been awarded restitution, a refund, or other monetary award by an arbitrator or court, based on a violation of this chapter by an institution or representative of an institution, but have been unable to collect the award from the institution.
- You sought legal counsel that resulted in the cancellation of one or more of your student loans and have an invoice for services rendered and evidence of the cancellation of the student loan or loans.
To qualify for STRF reimbursement, the application must be received within four (4) years from the date of the action or event that made the student eligible for recovery from STRF.
A student whose loan is revived by a loan holder or debt collector after a period of noncollection may, at any time, file a written application for recovery from STRF for the debt that would have otherwise been eligible for recovery. If it has been more than four (4) years since the action or event that made the student eligible, the student must have filed a written application for recovery within the original four (4) year period, unless the period has been extended by another act of law.
However, no claim can be paid to any student without a social security number or a taxpayer identification number.”
I acknowledge that I have read this agreement thoroughly, have received my copy and agree to be bound by it. I agree to abide by the rules and regulations described in the University Handbook and any other policies and/or procedures pertinent to my program/course of study. I understand that the refund policy is subject to change in accordance with Federal and State regulations and institutional policies.
I have had reasonable time to read and understand its terms, and that I have received: (a) written or web-based material which includes a description of the courses and educational services to be provided by Rocky Mountain University of Health Professions, including all material facts concerning the University and the program or course of instruction, which are likely to affect my decision to enroll; and (b) a written or web-based statement of the University’s Tuition Refund Policy. The Student’s Date of Enrollment shall be the postmark date under which this Enrollment Agreement was sent to the University.
II. Tuition & Fees
Program Enrollment Confirmation: The University’s acceptance of your application to this program does NOT guarantee you a seat in the program until you formally confirm your enrollment. To confirm your enrollment, submit your completed enrollment agreement, along with a non-refundable enrollment deposit, which will be credited toward your first semester’s tuition. Upon receipt of these two items, the University will send you confirmation of your enrollment in the program.

*Fees are subject to change. This is a standard enrollment agreement for all students. Any discounts or transfer credits you’ve received may affect your total cost and will be reflected on your individual tuition invoice.

III. Release & Waiver
As a student at Rocky Mountain University of Health Professions (RMU), I understand that I will participate in both required academic activities as part of my educational program, as well as optional extracurricular activities on or off the RMU campus.
Required Academic Activities
As part of my academic program, I recognize that there are inherent risks associated with attending a healthcare program where students may engage in practical, hands-on learning experiences. These activities, which may include but are not limited to, clinical practice, laboratory exercises, and peer-to-peer skill demonstrations, carry certain risks. I acknowledge and willingly accept these risks, which may include physical injury, discomfort or pain, allergic reactions, infection or transmission of disease, and emotional or psychological impacts. I understand that these activities are required for my educational development and I voluntarily assume responsibility for my participation in them.
I am aware that videotaping for educational purposes may occur during my attendance at Rocky Mountain University of Health Professions. My appearance in any video indicates my approval of its use. I understand that I may move out of the camera shot at will if I prefer not to appear on camera.
Any materials made available to you through Canvas are property of the University and are protected- including class video recordings.
If access is given to class recordings, the student agrees that the recording of the course will:
- be used only for meeting the academic objectives of the Rocky Mountain University of Health Professions course listed above;
- not be copied for any reason; and
- be viewed privately and by the student only
Any class recordings will be available for the duration of the semester in which the student is enrolled in the course. At the end of the semester, file access will be removed.
This Release and Waiver is intended to be as broad and inclusive as is permitted by law, and if any portion is held invalid, the balance shall continue in full force and effect.
By signing below, you acknowledge that information in a remotely attended session or the recordings of course sessions are the property of Rocky Mountain University of Health Professions and agree to all terms listed above.
Optional Extracurricular Activities
I further understand that I may voluntarily participate in extracurricular activities or events organized by the University, its affiliates, or individuals such as fellow students. Participation in these events is entirely optional and is not required as part of my academic program. These activities may involve physical, social, or recreational components, and I voluntarily assume all risks associated with them, including potential physical, emotional, or psychological harm.
By participating in both academic and extracurricular activities, I recognize that I am responsible for my own safety and well-being. I hereby waive any and all claims or rights to specific notice regarding potentially hazardous conditions related to these activities.
Release of Liability
I, individually, and on behalf of my immediate family and/or guest(s), hereby release and discharge RMU, its directors, officers, employees, agents, subsidiaries, affiliates, and other associated parties (collectively referred to as the “RMU Parties”) from any liability, claims, demands, costs, or expenses related to injury, loss, or damage to my person or property, or to that of my family and/or guests, arising from participation in any university activity, whether academic or extracurricular.
Furthermore, I agree not to pursue any future claims or legal action against the RMU Parties for injuries, whether known or unknown at the time of signing, related to my participation or that of my family and/or guests in these activities.
I HAVE READ AND FULLY UNDERSTAND THIS RELEASE OF LIABILITY AND WAIVER OF CLAIMS. I SIGN THIS DOCUMENT VOLUNTARILY AND UNDERSTAND THAT BY DOING SO, I AM WAIVING CERTAIN LEGAL RIGHTS.
IV. University Handbook, University Code of Conduct, and Orientation Review Acknowledgement
I agree that I have received, read, and understood all information contained in the Rocky Mountain University of Health Professions University Handbook. Subsequently, I also agree that I will adhere to and abide by the rules and regulations contained therein, which include, but are not limited to, the University Code of Conduct. I am aware of the consequences of violations of specific policies and standards, including plagiarism and dishonesty. If you are an international student, you confirm that you have read and understood the International Student Handbook. I also acknowledge that RMU 101 is a required orientation course that I will complete prior to the semester start.

V. Media Release & Waiver
I understand that my picture and/or voice recording may be in photos, video, audio recordings or other media intended to be used by the University for the use of marketing, promotion, events, publications, and any other use pertaining to the mission of the University. These may include but are not limited to websites and other internet mediums. This Release and Waiver is intended to be as broad and inclusive as is permitted by law, and if any portion is held invalid, the balance shall continue in full force and effect.
Please indicate below whether or not you give consent for RMU to use your picture or voice recording in the University media.
VI. Social Security Number Collection
Your personal Social Security Number (SSN) is requested for the following reasons:
- Identification and verification of student record data
- Compliance with Federal tax regulations related to annual IRS 1098-T form reporting obligations specific to student tuition payments
- Administration of Federal and State programs/loans, including Federal Financial Aid and other private loan programs
- Management of veteran benefit programs of third-party scholarship/grant programs
- Preparation of the student enrollment report required to be submitted by the institution to the National Student Loan Data System under Federal Law
- Submission to other Federal and State agencies as required
Please note that failure to provide the social security number may result in delay and confusion regarding identity, delay or loss of federal and state financial aid, inability to utilize certain federal tax credits, student loan deferments, veterans benefits, and other benefits under current law.
*US Social Security Numbers are not required for international students who do not have one.

By typing my name below, I acknowledge that I have read this agreement and the RMU Student Handbook in its entirety. I agree to the tuition and fees as outlined above and to pay a non-refundable enrollment deposit that will be counted toward your first semester tuition.
